Population Health Clinical Pharmacy SpecialistWe are seeking an experienced Population Health Clinical Pharmacy Specialist to provide clinical pharmacy services that improve medication outcomes, support chronic disease management, and promote cost-effective patient care.

Key Responsibilities:

• Review medication regimens and identify opportunities to improve therapy.
• Provide medication management and patient education.
• Support chronic disease management and population health initiatives.
• Collaborate with physicians and healthcare teams on medication-related care.
• Monitor medication effectiveness, safety, and adherence.
• Document clinical interventions and patient outcomes.

Qualifications:

• Doctor of Pharmacy (PharmD) or equivalent pharmacy degree.
• Active Pharmacist license as required by state regulations.
• Clinical pharmacy or population health experience preferred.
• Strong knowledge of pharmacotherapy, medication management, and chronic disease care.
• Excellent clinical, analytical, and communication skills.
